Rates
$150 per 43-minute session CPT code 90834, Play Therapy Clients and Parents
$200 per 60-minute session Individual CPT code 90837,
$200. per 60-minute session Couples CPT code 90847,
$300 per 90 minute session Couples and Individual.
Insurance
I do not process insurance claims. I am an 'out of network' provider, which means the process
follows this protocol:
I provide you with a therapy session or an assessment, you will pay me, I will give you a receipt
for the services that I provided you, and then you will file for reimbursement (since you paid me)
with your insurance company if you have the coverage for the services that I had provided for you.
Services may be covered in full or in part by your health insurance, employee benefit plan, health
savings plans, and flexible spending accounts.
Therefore, it is best for you to call your insurance company and ask about your coverage as well
as their procedures in filing.
Please check your coverage with your insurance company carefully by asking the following questions:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- How many sessions per year does my health insurance cover?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
Payment
PayPal, Venmo, Health Savings Account Cards, Flexible Spending Account Cards, and all major credit cards accepted for payment.
Cancellation Policy
If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the full cost of the session.
